I was alerted today that the report forms are not working. You're being asked for a password. I'm working with my web hosting tech support right now to try to figure out what's going on. Everything was working fine yesterday. In the meanwhile, submit your report by e-mail or hold it. I hope to get this fixed shortly. Sorry for the inconvinence.
