The squiggles aren't necessarily a sign of photoshop doctoring. Rather they look like block compression (e.g. JPEG) artifacts. As in this picture they are usually most visible in/adjoining areas of very high contrast/detail against flat/smooth areas.

My guess is real... Not Photoshopped.
