I do not think it is big Pharma perpetrating a grand scheme upon man. It is the nature of people that have created a business for big pharma. It is hard to eat the right things in moderation and exercise everyday. So the fat guy would rather take a hand full of pills. Its hard to accept your own mortality so people will do anything to stay alive even live in misery their lat 6 months than face the inevitable. I think Dr's know this about people and can tell in a quick visit that the guy that is 100lbs over weight and drinks too much and never exercises is probably not going to change his ways. So he tells him the truth and then gives him options the easiest being the pill route and the fat boy makes his choice. Even if there were no pills I doubt fat boy would stop killing himself a little bit at a time.

Aids for the most part is a lifestyle choice. Completely voluntary unless you are a woman and live in the wilds of Africa
