Been there before. Couple of suggestions.

No cotton. Anywhere. All polypropylene (sp), wool & gortex or good rain gear.

Instant hand & boot warmers. you can also get a product called Thermo care which are large instant warmers for the back etc. - they work great & produce alot of heat.

Even though their core temperature may be hot, if the ladies nose is cold you'll here about it. So go get her a nice face warmer like those ski masks.

A big wide brimmed Outdoor Research rain hat.

The warmest fluffiest boots & socks you can find.

Insulate her bottom from the seat too.

Hot cocoa.
