if you are fishing the wall off Tofino and are using whole horse herring you better have some slip leaders or your trailing hook will be half way up the herring. I'll use whole, black lable size with the 5/0s spaced as said. My tow hook is in the middle side back or further. Some of these fish have 10-11inch sardines as stomach contents. I'd say hook placement vs top of the bait.
